The Gratiot County Michigan Health Needs Assessment Survey and Analysis was conducted March through September 2002. The schedule maintained for this project is presented below:

• Survey instrument random household mailing: October 2002
• Respondent survey instruments returned: October 2002 - January 2003
• Primary Survey Method: 8000 survey instruments mailed
• Random mailing to homes in Census Tracts 1 through 9
• Number of Forms returned : 1100 (approx)
